How to trigger a notification upon termination, based on a Document Record being held.

Some of our employees hold PVG (Protecting Vulnerable Group) certificates, issued by Disclosure Scotland. If any PVG holders leave the organisation, we are required to notify Disclosure Scotland. I'm looking for a solution that will notify HR when a leaver is a PVG holder. I was thinking that the PVG could be held as a Document Record and was wondering if there is a way to configure a notification that flags when a terminated worker has a document recorded against that particular Document Type. Has anyone done something similar or does anyone have an idea about how this could
