How to create Approval Workflow for Disabilities

Summary: We have a requirement where we need to create an Approval Workflow for Disabilities. Like when an employee is selecting 'Yes' or 'I don't wish to Answer' for Disability it should go for approvals. But I am not able to pull the field for Disability in IF condition. I have checked PersonInformationRule and ManagePersonRuleSet but have not been able to find any field for Disability.
