Initiative with survey option enabled is sending qualifications to the same internal responder twice

Summary: I have created an initiative with internal survey option enabled, added default internal responders, then launched the initiative. I was surprised that the system generated 2 different qualifications for the same internal responder. Therefore if I had 3 default internal responders for one supplier, the system will generate 6 qualifications. 2 for each internal responder.

7- launch initiative, system sends 6 qualifications, for each internal responder he/she needs to answer 2.
