Error while importing Location file in FSM

Summary:

When I am trying to import Location file to create location in the test instance, I am receiving below error (PFA error file excel & screenshot as well)


Can someone help me to understand where the error is? I am also attaching the file which I am trying to import in ZIP format

Content (required):

I am trying to upload bulk locations (count: approx 300) in the instance. I am trying one location first but getting the same error again and again.
