Transfer Order not getting created for IMT flow

While creating an Internal Requisition, no Transfer Order is getting generated due to the following error. Has anyone faced this problem earlier? Any directions or inputs please?

The supply order creation failed because the source organization, source subinventory, or both are invalid

The Supply Chain Orchestration doesn't show any details / failures regarding the Requisition in failure. We have tried searching by Requisition# and Item code.
