Page name not appearing person extra information

Summary:

Hello I have created a page and I can see it when I first go to Person Extra Information:

I can add to it as configured.

However, the name of my page is not appearing on the left side w the France and England ones. So if I click on one of those, I can never get back to my BDS page:

Also, when making the 'unique field' is there anyway to have it auto-populate. Or maybe it's good to use the date as the unique ID?
