I understand Oracle states it is best practice to only have one item master but can someone tell us what the down side it to multiple item masters?

I understand Oracle states it is best practice to only have one item master but can someone tell us what the down side it to multiple item masters? As we move towards hosting a rural hospital that we do not own it makes sense to us to build them their own item master. What are the considerations and risks in doing this?
