Approval history is not available on notification&transaction console

Summary:

Hello,

Approval history is not available on the notification (when line manager or HR clicks on the link to open the notification and also on the transaction console).

We are not able to see details of the notification such as attachment/comment... added by employee due to the history not available.

Has anyone encountered this problem before? How did you solve it?
