How can I modify the Electronic Record for Inventory Miscellaneous Transaction Report?

Summary: I need to add the Reference and Reason field to the RTF Template so that the approving user can make an informed decision without calling the user requesting the transaction.

Content (required): The RTF template for the Miscellaneous Transaction Report doesn't contain all the necessary fields for the approving managers to know why they are approving inventory issues or receipts, that's why we need to add two extra fields to the template, however while trying to get an xml from the data model there's no available information.

Anyone knows how I can modify this?
