How to remove the voluntary self-identification form (CC-305) from US disability info?

Is it possible to maintain the ability to report and track disability details, and any corresponding accommodation requests for the United States, without having the Voluntary Self-Identification of Disability appear?


The form states that the employer is a federal contractor, however is it advisable to collect disability info for employers that are not federal contractors nor subcontractors? Or should all employers be using this form, despite the fact that the language is specific to only contractors and subcontractors.

Update:

Confirmed that only federal contractors should be using the CC-305 form. The interest however to enable the ability to provide a self-service way for an employee the means of requesting an accommodation continues - it seems like most users in the community have created a custom solution for this use case case. It would be nice to see Disability info expand to support accommodations beyond only disabilities since request requirements can relate to disabilities as well as religious needs, etc.
