How to get Questionnaire Answers on a Single Row in OTBI

I have created an analysis using the Workforce Management - Checklist Real Time subject area that is pulling questions and responses for a specific journey task. The task is configured using a questionnaire. The questionnaire has 5 questions. The first 2 questions are text box single response questions, the 3rd question has 3 possible answers and based upon the answer selected, 2 additional questions open up.

In my analysis, I created a case statement to order the questions how they appear in the UI, I also added the check, that if the Question Text is Null use the Response Answer field. I created a pivot view so that I can display the questions as column headings with the answers below.
