How to link Allocated tasks with questionnaires

hello everyone,

In order to create a report on the offboarding /exit interviews i need to link the allocated tasks with the questionnaires to get the allocated tasks and the answers for each questions in those forms and questionnaire.

At the moment using Allocated tasks I can only get the informations about who's the person that have allocated tasks and which is the actual status, but the main need is to get this informations combined with the questionnaires tables in order to get the full details the tasks, the employees concerned and their inputs and answers for each allocated task
