Need the ability to see who has deleted updated or added a row via Change Salary

Content (required):

I am looking for a report that will show me everything that has happened to a users salary history. I need to be able to see when a row was added (and what was added), when it was changed (and what the changed values are) and if it was deleted (and by who). This report should show each and every time it was modified. If someone has a salary change row effective 10/25 and it was modified by HR 3 times in one day, there should be 3 rows showing the individuals that each made the
