Can I add a form into an absence?

Hi team,

Due to the government regulations, for each absence added on a specific country, there is a different documentation that needs to be uploaded as proof of absence.

Is there a way I can add an absence from to be accessed and completed by the employees, when they are requesting an absence that will guide them to the correct documentation to be attached based on the absence that is requested?
