An error code while load the grade file

I would like to update the grade to workers using HSDL

I tried to use the following file-

Those fields are the 'must' when I choose grade code in the definition section

after I upload I get the fowlloing error-

What is the parent record? assignment? what do I need to do to succeed upload this file?
