I would like to hear your ideas for implementing an employee performance review in a simple way.

The steps that are needed are- 1. A manager fills out questions ( I have these questions in Word Docs) 2. The employee fills out question 3. The manager and the employee meet, and then the manager must fill in some more questions 4. The manager of the manager needs to approve the review they fill and answer the question for him

The HR team needs to have access to see what's the status of the process
