Assign Learning to my Team Issue

Summary:

Hi Experts,

We have observed that when manager assign learning to his team , certain times the process fails out for certain individuals. One of the cause is that same learning item might be assigned to learner. I am looking for all the possible reasons why the process fails out or any oracle documentation on this process.
