How can we pull data in custom report for the field name "Action" on Termination page.

Summary:

I am trying to generate a custom report and want to know the column name for the field name “Actions” in the Termination page.

We do get the data for the other two fields i.e. "Termination Reason" & "Termination Comments" but no option available to choose "Action" field.
