Record Issue feature is not working

I tried to record several issues but the feature is not working correctly. The recording starts and the error message will flash across the screen. After the error message flashes, I stop the recording. Then, one of two scenarios happen:

(1) The Finalize Recording window appears and there is no data - no Recording Number, Server Domain, Server Instance, Application or Page Name. When I go to View Recordings, the recording does not exist.

(2) The Finalize Recording window appears and data is populated. I click Submit and then go to View Recordings. The recording is in on the list of recordings but it is either already purged, or purges within a couple of hours.
