Getting Error when adding Questionnaire Text and Response Text for Observer Checklist reports

Has anyone put together a step by step in creating the Observer Checklist Responses? I can't find all of the options listed in the Use Case and even trying to run it on most the report runs and runs looking for data for a long time (over 10 minutes) and then comes back with error.


Update: I started slowly and was able to locate the line the Activity happens on, but as soon as I try to add the Questionnaire Text and/or Response Text the report just goes into "Searching..." for a long time. Before adding this column it would come back with my result within about 10 seconds. Now it just spins and searches for about 15-20 minutes. Then it comes back with the error in the attachment.
