When employee takes an assessment and evaluation, "No Answer" option is showing in each MCQ

Summary: When an admin creates a questionnaire as required, "No Answer" option is not showing but when an admin creates a questionnaire as not required, "No Answer" option is showing in each MCQ. I have attached a screenshots. This option should not show when an employee gives an assessment and evaluation.
