Hi,

we would like to have the possibility to enter a survey/questionnaire inside a document of record. In other words, when an employee goes into Me -> Personal Information -> Document Records and selects a specific document, in the document selected he/she has to view a Questionnaire/survey he she has to fill in. We cannot use flexfields, because the survey contains conditional questions (basically the second question should appear, based on the answer given to the first question and so on).

Hoping someone has some suggestions.
