Strange UI Bug - See screenshot

It was easier to just show this then try and explain it in the title. A user is seeing the below. Everything I circle in red is out of the ordinary. There is no Icon for the navigator, there's a weird "Item" menu at the top of the form, Instructions is spelled out next to the "i", "Item" and "Selected item" is in the various card selections.

This was occurring for the user in both Edge and Chrome.
