How to add a text box in the dropdown list when option 'Other' is selected? - Personal details tab

Me - Personal Details - Demographic Info - Gender Identity - Religion - Ethnicity

Long story short... I created a new Lookup Code under ORA_HRX_GB_GENDER_IDENTITY. A new LookUp Code name is: Other (Please specify) and I would also like to add a text box so if users select option Other they can add/type their comment.

The same dropdown filed - Other (Please specify) will be added to other categories.
