clicking Approve still gives attachment pop-up even when turned off-Workflow Notifications in Bell

Summary: This is regarding time card approvals. When an employee submits his/her time card, a manager then receives an approval workflow notification. A manager enters the Bell Icon, clicks on the request, then has the choice to Approve / Reject. When the Approve button is clicked (or even reject) a pop-up appears with a prompt to add attachments. Why? We have attachments off in the BPM config:

Content (required): We want a manager to click Approve, see no add'l pop-up, and be done with the request/workflow notification. What are we configuring incorrectly? Thanks.
