Data discovery not creating notes attachment in the form

Summary:

Hello Gurus,

I am using data discovery to trace my value populated in a combination.

Post to execution of Data Discovery in Data form, I am unable to see the Notes attachment. anyhow the data grids for the data discovery got created in Data card section.

Please help me if I need to do any settings.

I want to know the source (Rule or logic) of the value, which was not input or load as per data audit.
