Workflow Failed: Approve Performance Goal

We have a transaction failed for 'Approve Performance Goal' transaction and the system says that the solution is assigning a supervisor to the user. However, the affected user has a line manager maintained in the system. We have already tried to 'Recover' the transaction but it is getting failed.

FYI: we have the Refresh Manager Hierarchy job scheduled in the system.

Do you have any idea why this specific transaction is getting failed? Because we can see that for the remaining users, it is working fine.
