MWA has been turned on for 4 years, but recently it doesn't seem to be working correctly


Is anyone else experiencing this issue?
We have "allow multiple attempts" turned on, and have had since we launched four years ago. (There have been no changes to those settings)
Recently we started receiving complaints from users that the system is not recording new completions of a course. 

 For example:  I can see that I have completed course A on three separate occasions and have 3 different completion records.  But if I complete course A today, it doesn't show today's completion.  It only shows that I "accessed" the course today.   We have to use Bulk Loader in order to add the additional completion record.
