Content files randomly go missing

Has anyone else experienced content files that randomly go missing?
Content (required):
After wondering if we are losing our minds, I have to ask if anyone else sees content files just suddenly missing. We normally do not use any expiration dates on content files, so it would not be that. And it is not any specific type of content. Yesterday we found an offering that has been working just fine suddenly is missing its content. We have multiple admins and all of us have experienced the same thing .
When this happens we see that learners have been completing just fine, then will get a call about a course not working. When we track back, we find that the content file is no long anywhere to be found.
