Evaluation Report is not shown Answer Text and Comments attached to Answers

Summary:

I created an Evaluation Report with following details like Question Text, Answer Text, Response List ShortText, Response Status using Learning Managment Real Time Subject Area. But I could not see values under Answer Text, Response Status showing submitted though it was not answered.

Can anyone help how can we get values of Answer Text, Comments I submitted for each answer.

Note: Question that I created is a single choice which allows comments for answer.

Question: The goals of the training were clearly defined

Answer: Strongly Agree

Comments: Agree to this question

Do we have choice of printing this in the report, it would be great if you have any Sample Reports, please share. Thanks.
