Is it possible to add a comments section to document types?

Summary:

Our HR team would like to be able to add comment sections when approving documents in the "Document Details" section. We already have a couple of documents that have this functionality but would like to add it too more. This would allow HR team members to provide employees for reasonings as to why their document was approved or not. I have provided two screenshots of what we are looking to achieve.
