How to create a notification when a Salary change fails

Is there a way to create a custom notification to send to the manager or HR when a salary change fails?
Content (required):
We had a bunch of salary changes fail due to a workflow problem and no one knew it failed until an employee noticed the increase wasn't on his pay stub. It would be great to create a notification so we know right away when the action fails so we can correct it.
