Vehicle fitness form before starting the trip

Content (required):

Customer is looking for adding the vehicle fitness form that can filled in from OTM mobile application or the web version of app.

This is to be done before starting any shipment in order to track if all the vehicle parameters were fine before the trip and there was no problem with the vehicle. Just want to know how this can be accommodated via the Mobile app web version . If not is there any workaround or any modifications that can be done on the Mobile app.
