Document of Records - Attachment is not updated when we are trying to correct/update the data.

Summary:

we have created a custom report for Document of records and given the path at Document Types. But when we are trying to correct/update the data of existed transaction it is going for the approval and after approving the transaction updated values are showing at the UI level, but those updated values are not updated in the attachment level.

example: Attachment have been auto created after approving the transaction. while correct/update the values eg: from "ABC" value to "XYZ" the "XYZ" is updated in UI level but in the attachment level not updated. please provide me any workaround if a seeded behavior of the system.
