Learners are saying they've completed e-learning, but system shows status as No Active Offering

Hi All
We've had a few learners recently contacting us to say they have completed their mandatory e-learning, but when we check in OLC, their status is showing as No Active Offering. They are enrolled automatically at the course level via an initiative.
We do have instances where the SCORM tracking fails, but with those learners, they show as being enrolled onto an offering with a status of In Progress.
Has anyone ever come across an instance where a learner has managed to access e-learning, but OLC has failed to even register they have enrolled onto the offering?
I'm pretty sure our system is not the issue.
