How do i keep track of when a form is declined in a dynamic process

Summary:

I want to create a Resubmit Form Request which is triggered and sent back to the creator when either Approve Request from Cashes / Engineering is declined from their managers.

Right now, the form request goes to the final approval whether the first approval approves or declines. I need a way to be able to keep track of when the form is approved or rejected. What parameter can i track to create a data condition or event.
