Query showing the values on the current Tax Calculation Cards

Summary:

I need BI publisher reports that show the values on the current Tax Calculation Cards for all active employees including their hire date. The values of greatest importance to me at the moment are:

Federal:

Exempt from Medicare

Exempt from Social Security

State:

Exempt From Family Leave Insurance

Content (required):

I've had some success for Federal with the following query but I get multiple rows for each person number. The rows seem to be the same except for different ValueDefinitionStartDate  and/or assignement_number. I only want one row that shows the values for the current assignment and calculation card values. I also have been unable to get state values like "Exempt From Family Leave Insurance' and ideally would like them on this report if possible but having them on a second report with just state values would be adequate. I would also like the hire date on the report
