Does questionnaire deeplinks exists?

Summary:

I need to give users a link to evaluation questionnaire after completing a course. How to build it?

Content (required):

Under Tools > Deeplink I don't see references to questionnaires but it loads only 400 element out of 800 so I'm not sure.

Alternatively I'd like to know in which table is stored this link that I find under course activities:

For my point of view is enough to me to get an answer for just one of these things.
