How to user Action Reason for custom actions?

Summary:

We are trying to user Action Reason for custom actions.

The audit action was created uploading a CSV file so we could setup the action reason.

When the action is triggered it prompts the status code as expected. The point it is not recording it into table audit_trail. For standard actions it works fine.

Is there any way to make it happen for custom action as well?
