Summary:

We need to change approval workflow for User Item Type change order as below, without impacting other CO approvals-

 1st level of Approval - TC Warehouse Item Change Order Custom role

AND then 

2nd level of Approval - TC Hospital Pars Item Change Order Custom and TC Hospital INVs Change Order Custom in parallel.

We need BPM rule for configuration.
