EDM has recently started sending false email saying policy is invalid

Content (please ensure you mask any confidential information):

When a request is raised in EDM, it sends out a false policy error email like the one shown below. The said policy has a group with 2 users and those users are able to approve the request. The request also gets closed successfully after the approval goes through. We are not getting the reason why the below email notification keeps getting generated when the request is raised in EDM. Is this some sort of a bug?
