Deleted activity still requires completion

Summary:

An activity has been deleted from the offering, and replaced with a new activity. However the number of activities required to complete has increased by 1, and a learner is not longer able to complete all the activities. The deleted activity is not displayed.

Has anybody else experienced this. How did you resolve the issue?
