Task aggregation not working when set to None

Summary: Even with the Task Aggregation setting set to "None", when the same approver appears in the workflow the transaction is getting auto-approved if the approver has already approved once.

In the above Screenshot Amika is appearing thrice in the whole workflow, but after she approves the first time, the transaction gets auto-approved the next times as shown by the rows "Approved by workflow system"
