Do you have an assessment results report created with OTBI?

Summary:

I created an assessment using the questionnaire tools in Oracle. Does anyone have a report they can share to get the results of an assessment using the new subject area Learning Records Real Time? I'd also be happy to use the old subject area if that is what you have. Two pieces that are giving me trouble -

I'd like to see if the answer the learner gave was graded as correct or wrong by the system.

On the Multichoice questions I'm not seeing all the answers the learners gave. It looks like I just get the first answer they gave.
