Learning request approval is not triggering

Learning request approval not triggering for learners

Hello all,

I've set offering level Learner initial assignment status = requested (at offering level)

and used the following approval to trigger approval for courses

But approval is not triggered to Line manager, I also want second approver to be basis on AOR. Can anyone let me know what am i be missing?
