oracle learning fusion issue with retake button.

Summary:

Upon failing and clicking on the "retake assessment" button, the LMS is not able to capture the failed score and number of attempts. If the learner passes, it is considered as 1 attempt and the passing score is displayed. Could you please advise on what settings we need to adjust in the SCORM file or in the Oracle Learning Fusion LMS in order to capture the number of attempts and the score upon clicking on the "retake assessment" button? Notes:

 1)we have noticed that if we remove retake assessment button LMS was able to capture number of tries and score.
