Need HDL Template to load questionaire template with randomizing option as YES

We want to load the question with randomization option as yes.

Can anyone share the required filed we need to update in the HDL template for this requirement.

It'll be very help full if anyone shares the HDL file.

And also if we want to randomize only 5 questions out of 10 how can we achieve this through hdls
