why does the Plan Members Products doesn't match the Item catalog in PIM?

Summary:

Item catalogs in PIM are showing in Correct Hierarchy, and upon catalog collection in DM>run plan. the Item is showing in another Folder inside the catalog, and not in the same way it was created in PIM. how is this happening?
