TCF Initialization Failed in Request Change.

In Time and Absence, when an employee goes to the Request Change page TCF Initialization Failed is displayed and the custom fields are missing.

We enabled the Change Request option for custom fields in the Time Layout Sets. Please find the attached screenshots.
