Trying to build a HSDL to load questions and answers into question library.

I am receiving an error when I'm trying to load a question with answers. Do I need to create two templates? One for the questions and one for the answers or is there a way to load them all in the same template. I have attached the template and the error I am receiving. Any help would be greatly appreciated!
